What we do
- The CLDT helps people with learning disabilities find health and other services that can help them. We carry out assessments; this is where we talk to you and find out what your needs are. Your schools will tell CLDT that you might need services when you leave school.
- We will meet with you in the autumn term before you leave school to decide what services you may need. We will help you find things like a job, training, leisure, help in the home, respite care and assist you in using organisations like council services or community groups.
How to refer
If you think you need help from CLDT you can ask for it from the Duty Desk who will see how much help you need. (See telephone number above)
Cost
People may have to pay money for some of the help like homecare, day services, etc. CLDT works out how much money you can afford and looks at what benefits you are on.

People with the most need get help from CLDT first. Learning Disability Services have eligibility criteria that say what types or level of services you are entitled to. CLDT can give more information on eligibility criteria.
