What we do
- We are a flexible, creative, community based scheme. We support people to live in the community and provide a service to people with a learning disability and mental health problems aged 18 and over.
- We provide a service to all older people aged 65 and over.
- We can provide a short and long term breaks for people with a learning disability, mental health problems and older people.
How to refer
We accept all referrals via the Community Learning Disability Team or the elderly/mental health team. These teams carry out an assessment before referring.
