What happens when you begin to think about leaving school?
- You might have been assessed when you were younger and have a Statement of Special Educational Needs. Your parents (or someone else who knew you) probably asked for an assessment. Then a statement of your needs was produced. This said what the local education authority would do for you.
- This statement was reviewed every year until you were 14. Now it becomes the starting point for your Transition Plan. This should build on the previous reviews of your Statement of Educational Needs.
Your Transition Plan
- Around the time you are aged 14, your review meeting with the Local Education Authority is used to draw up your Transition Plan. This will help you decide what you will do after you are aged 16.
- The Education Department will write to tell you about a forthcoming review. After that, it is looked at every year to make sure it's up to date - it should bring together all the different help you might need.
Who might be at the meeting?
- People who might be at the meeting include:
- A teacher
- A social worker
- Someone from Connexions
- The relevant health authority staff
- You and your parents or carers!
What will be in the plan?
The meeting will probably be at school, but it isn't only about education. It should cover your other needs too.
The plan will look at:- What you want to do
- Where you will live
- What information you need
- What support you need to decide about your future
- Any special equipment and personal assistance you need
- New skills you need that your school can help you with
- What you need to become independent and confident
- Your social life
- Using leisure facilities
- What your health support needs are
It's your life!
- It is your right to be listened to and you can communicate in the meeting how you like: using symbols, sign language or whatever you need. You can also see any relevant reports before the meeting.
- It is your life so your views count. It's worth thinking before the meeting and making your own plan so you are prepared.
