Community Learning Disability Team
CLDT is a joint team made up of staff from Greenwich Adult Services and Oxleas NHS Trust. Find out their work by downloading this leaflet (pdf 244kb).
The team finds the right support and services to meet your assessed needs.
- Care managers assess people's needs, and then work with people to plan, set up and review care to make sure people get the support.
- Community nurses work with people to support their health needs. Find out more about their work with personal health profiles in this leaflet (PDF 337kb).

Person-Centred Planning support people to manage their own person-centred plans.
- Physiotherapy staff work with people that need specialist support to maintain and improve their mobility.
- Speech and Language Therapy support people to make their communication skills better. They also work with people who have difficulties eating.
- Occupational Therapy assess and support people to maintain and maximise their independence both in their homes and in their local communities.
- Psychology work with people who are finding it difficult to cope with things that going on in their lives.
- Psychiatry work with people with learning disabilities who have mental health needs, or who need specialist support with things like making sure their medication for epilepsy is correct.
